	Services Benevolence Fund Our church loves people and as a way to help we have crafted a policy and application that allows us to lend support for individuals and families that might be struggling. Please fill out this application form, submit it, and we will respond to you within 7-10 business days. Application Pre-Marriage Counseling This is a questionnaire that is to be filled out individually by prospective spouses. These questions will be shared during pre-marital counseling sessions. Please have these filled out, printed off, and submitted before your first scheduled pre-marital counseling session. Questionnaire Wedding Policy & Application God has blessed us with a gorgeous facility to host weddings. In a desire to best accommodate this special occasion we have a policy in place to guide this God honoring commitment between a husband and wife. Please read this policy over before filling out the application. Every wedding held at Calvary Bible Church needs to be pre-approved. We ask that anyone who is interested in having a wedding at our church facility please fill out this application form and submit it to our church leadership. Our church leadership will review each application and we will try to schedule an initial consultation with the prospective bride and groom before approval is given. Policy & Application Facilities Usage Policy A variety of events from Baby Showers to Anniversary Parties to Banquets, etc are welcomed to use Calvary Bible Church’s facility. In order to keep a consistent, well-maintained resource we ask that anyone interested in using our facility please read the policy, fill out and submit the application. 	Kids Child Information Children are a gift from God and we desire to protect and provide a safe environment for the children that are entrusted into our care. To help to accomplish this we ask that a parent or guardian please fill out this Child Information form for our church’s personal records. Calvary Bible Church will hold onto these forms and ask if there are any changes that a parent or guardian would resubmit any changes so that our records are current and up to date. Information Northern Frontier Registration Boys 8 years old through 16 years old as well as fathers/sons can experience a variety of wonderful programs that enjoy the splendid creation of the Adirondacks. Our friends at Northern Frontier point these boys and young men to Jesus Christ with each activity they encounter. Calvary Bible Church encourages boys, young men, and fathers/sons to embark on a week or two at this wonderful Christ-centered camp. More Information Register Now Word of Life Ranch/Wild Registration Children 1st grade through 8th grade can participate in one of these fantastic camps. Both the Ranch and the Wild are located at the same facility, but offer two totally different age-appropriate experiences. Each of these camps focus on exposing youth to a personal relationship with Christ, growing in that relationship, and impacting their areas being on-fire to share the Gospel with their spheres of influence. Our friends at Word of Life do a tremendous job of mixing fun activities, great food, and the life-changing message of God’s Word. 	Youth REVERB Registration This all-nighter event is a mix of a hockey match, high energy competitions, great prizes, pizza, bowling, other exciting activities, and a clear Gospel message. Each year Calvary Bible Church takes a bus load of 7th through 12th graders to this incredible event. If you are interested in coming with our church to this year’s REVERB, please fill out the registration form and submit it. Each year Calvary Bible Church takes a group of 7th through 12th graders to this winter weekend event. Our friends at Word of Life specialize with youth ministry and we encourage all teens to experience a week or two at this wonderful Christ-centered camp. This is a fantastic opportunity for our young people to learn new skills and minister to another Christian organization. In the past we have helped our friends at Camp of the Woods and the Jesus is Lord Camp. 	Missions Missionary Policy Our missionaries are an extension of church family. This policy explains the commitments that we as a church make to those we support on the mission field. It also describes the various ways that make it easier for our church to appropriately pray for and support our missionaries. Policy
	Adult Men’s Retreat Men of all ages are encouraged to join this getaway that focuses on issues that men face. Not only is this a great opportunity to learn and grow, it is a splendid chance to make new connections with other godly men. It is a great opportunity for women to rub elbows with each other and worship God together. Many laughs and memories will be shared on this getaway that will help you grow in your walk with God. Jesus Is Lord Camp Registration The Jesus is Lord Camp is set in a gorgeous environment and on an amazing lake. Prepared Meals Request People in our community might be facing life transitions or difficulties. We care about each person and in those situations that are challenging we offer frozen prepared meals. There are a variety of meals available each month. If you wish to get a frozen prepared meal please fill out the request form below and submit it. Word of Life Devotional Order At Calvary Bible Church we believe reading God’s Word is critical to our personal growth. Word of Life puts out a one year daily devotional that ranges from 10 to 20 verses a day. There are age appropriate devotionals available for purchase.
